Grant Reports and Change Requests
Grant change request
Please submit a grant change request form if you would like to request an extension to the grant period and/or request a budget adjustment of more than $500. Grant change requests are approved if the request does not change the nature of the original grant purpose. Extensions of time are generally granted for six months with a completed grant report due at the end of the six month extension. Budget adjustments greater than $500 require approval of the foundation.
Submitting the grant change request
Complete our grant change request form (MS Word).
Email the document and include "Grant Change Request" and the grant ID in the subject line to firstname.lastname@example.org.
Final and interim grant reports
Grant recipients are required to complete a final report upon completion of the grant term (typically 12 months) before subsequent grant applications will be considered. Recipients of multiyear grants must submit an interim report on the anniversary of the original grant award date before additional funds are released. Please note on the grant report form which type you are submitting.
Information to include in the grant report
- We rely on your candid grant reports to assist us. Please give us your honest thoughts about the short-term results and the longer-term potential of the project, program, or work for which funds were granted.
- We are just as interested in what didn’t work as what did. While we understand the wish to focus on successes, we respect the candor and insight involved in also sharing hard lessons learned.
- Your grant report should be based on the description and evaluation plan in your original proposal narrative. Please refer to your original proposal as you prepare your grant report.

Submitting the grant report
Email reports (strongly encouraged) and include "Grant Report" and the grant ID in the subject line.
For Tillotson Fund grants, please email your grant report to Jean Clarke.
